Welcome to the Smooth Transition website. Terri wrote the transition booklet with the encouragement from many health professionals, social workers and teens with disabilities. She gathered information from her personal experiences regarding the transition process from teen to adulthood for people with disabilities. Terri hopes you will find this resource to be helpful in planning with confidence and effectiveness.

Mission

To assist teens, parents, doctors and other medical professionals, social workers, vocational rehabilitation centers and anyone involved in the transition process for teens with disabilities. It is my desire for every special needs teen to have this valuable guide in hand by the beginning of puberty, so the transition process becomes a natural part of his or her everyday life. Ultimately, the end result will be a smooth transition into adulthood with the knowledge and confidence required to be successful.
